Policy Provision

I. Contestability

Natural Death

Death due to natural sickness requires a 1-year contestability period from the date of effectivity or last reinstatement of the policy. Death due to a driving without a driver's license or expired vehicle documents is considered natural death. Death due to unprovoked murder or assault is also considered natural death.

Accidental Death

Death due to any form of accident requires a 7-day contestability period from the date of effectivity or last reinstatement of the policy.

Upgrading of Coverage

The effectivity of new coverage requires a 1-year contestability period from the date of upgrading.

Suicide

The company will not be liable if the insured member dies by suicide within two (2) years after the effectivity date or last reinstatement of the policy.

II. Grace Period

Members are given a period of sixty (60) days or two (2) months from the due date to renew their plan. If a member fails to pay after a certain period, it will be considered lapsed. Anything that happens to the member after the grace period means the company is not liable for any claims. A member may reinstate their account provided they pay two (2) months and a reinstatement fee.

III. Notice and Proof of Claim

Written Notice of Claim must be submitted to the company at its Home Office, or at any of its authorized offices, within thirty (30) days from the date of the member's death. The company must then provide the claimant with the necessary form for filing Proof of Claim. All Proofs of Claim must be received by the company within sixty (60) days after the date that the forms are provided. Failure to submit the written Notice and Proof of Claim within the limits will invalidate or reduce any claim if it is shown not to have been reasonably possible to give such Notice and Proof and that such Notice and Proof were given as soon as reasonably possible.

IV. Reinstatement

The member has the privilege to reinstate the lapsed membership. However, the member may reinstate their account provided that he/she will pay the reinstatement fee amount of (P500.00) Five Hundred Pesos and the member reinstates his/her account to the prevailing price.

V. Benefits

In case the principal member dies in Natural death or accident it will receive the memorial package and cash assistance benefits. There is a 12-month contestability period for natural death and 24 hours for accident from the inception of the contract or from the date of the last reinstatement. In the case that the family members died in natural death or accident they will receive cash benefits only but convertible to memorial services.

VI. Memorial Services Provision

As to the specific arrangements for your package, exceeding kilometer will be charged by our funeral services chapel payment arrangement will be settled between the family of the member and the funeral chapel. A pre-arrangement and set up of our package charge for additional number of days should be arranged between the family member and the funeral chapel. The package inclusions are: 1. Coffin, 2. Flowers, 3. Earthen pot of Rice (Optional), 4. Decoration, 5. Chosen Casket, 6. Cabbage (Optional), 7. Burger (Optional).

VII. Conversion Privilege

Any member has the privilege to convert the coverage package: Cash - Memorial Services; Memorial Services - Cash. Any conversion of a package coverage is subject to a corresponding deduction percentage.

VIII. Transferable

The member has the privilege to transfer the coverage with any living person. However, a new member is subject to a period of contestability from the date of transfer. All rights and coverage will be transferred to the new member.

IX. Assignable

Payment of Service balance/assigned account should be made on or before interment. A Letter of Authorization for the transfer of the assigned package will be submitted. The transport of cadaver from point of origin of destination is not shouldered by the company.

X. Yearly Renewable Term

The member must renew the charge to renew on or before thirty (30) days from the due date. Failure to renew in a certain period, the plan will be considered lapsed. Lapsed accounts are subject to another additional six (6) months contestability period.

XI. Hospital Daily Allowance

A hospital daily allowance is a Yearly Renewable Term-Insurable. With a hospital daily allowance for a regular membership 5 days each family member per year with contestability of 3 months from the effectivity date and activated while paying the period only.

XII. House Fire Protection

A house fire protection for a yearly renewable membership. With a house fire protection for a regular membership only with a contestability of 3 months from the effectivity date and activated while paying the period only.

Service Agreement

Authorization to receive benefits for memorial services. The undersigned hereby affirm their full legal age and residing at the indicated address. The owner of a Membership agreement issued by Family Life Care Int'l Insurance Services do hereby authorize to allocate the amount for funeral service that may be rendered by a designated funeral service provider. Family Life Care Int'l Insurance Services reserves the right to cancel and/or reserve the right to make all negotiation and necessary arrangement with a memorial servicing tie-up of its choice with the memorial service provide for this agreement.

I hereby certify that I read and fully understand the terms and conditions regarding my membership in FAMILY LIFE CARE INT'L INSURANCE SERVICES and all of the information given in this application is true and correct to the best of my knowledge.
